Explicit Consent Notice for the Processing of Personal Data

I declare that, within the scope of the Personal Data Protection Law No. 6698 ("KVKK"), the necessary information has been provided to me by GoWellFly via the Information Notice on the Processing of Personal Data, and that I have read and understood this notice.

Within the scope of KVKK, explicit consent means consent regarding a specific subject, based on information and disclosed with free will.

In this context, I accept, declare and undertake that I give my explicit consent regarding the matters set forth below:

1. General Declaration of Explicit Consent

By GoWellFly;

my identity information
my contact information
my usage and transaction information
my device and technical data

I give my explicit consent for the processing, storage and, where necessary, transfer thereof in accordance with the purposes set forth in the Information Notice.

2. Special Categories of Personal Data (Health Data)

In the event that I share via the Platform on my own initiative:

my health information
my treatment history
my medical documents

I acknowledge that the foregoing constitute special categories of personal data within the scope of Article 6 of the KVKK.

I give my explicit consent for these data to be:

shared with the relevant clinics via the Platform
processed by the Clinics
used for the purpose of carrying out healthcare service processes

as set forth above.

I further accept that:

the data controller for these data is the relevant Clinic
GoWellFly is solely a technical infrastructure provider

3. Transfer of Personal Data

I give my explicit consent for the transfer of my personal data to:

the clinics on the Platform
business partners
technical infrastructure providers
cloud service providers

as set forth above.

4. Transfer Abroad

I acknowledge that my personal data may be transferred abroad via:

cloud systems
international service providers

as set forth above.

In this context, I give my explicit consent for the transfer of data abroad.

5. Explicit Consent for Artificial Intelligence Systems

Within the scope of the artificial-intelligence-supported systems offered through the Platform, I give my explicit consent for:

the analysis of my data
the creation of profiles
the use thereof in recommendation systems

as set forth above.

I accept that:

these systems are of a supportive nature only
they do not replace medical diagnosis or treatment

6. Marketing and Communication

I give my explicit consent for the processing of my personal data for the purposes of:

campaigns
information
promotion

and for the sending of electronic messages to me.

7. Withdrawal of Explicit Consent

I acknowledge that, within the scope of the KVKK, I may withdraw my explicit consent:

at any time
without giving any reason

as set forth above.

8. Declaration

Within the scope of all matters set forth above, I declare that:

my explicit consent is based on information
it is given with my free will
I am not under any duress

as set forth above.
